You cant put out the flames at olm, it does the animation of casting a spell but it doesn't extinguish any flames.

In the thieving room after the scavanger has enough grubs to go to sleep, you should still be able to put grubs in the trough. You just shouldn't get points for it.

The braziers go out way too quick in comparison to OSRS at ice demon, this makes soloing it without a storage chest near impossible. (Found this bug after some testing with @Joeyrocks130)

You should be awarded Firemaking XP while lighting the braziers, atm you don't get any XP.

If you stand under Mommadile she completely deaggros you, you can barrage her while standing under her and she won't attack you.
